Auf dem CD-Cover sieht man im Vordergrund eine illustrierte Gestalt mit langem, weißem Haar. Das Gesicht der Figur weist eine rötliche, muskelfasernartige Textur ohne Haut auf, wobei die Zähne freiliegen und Blut aus dem Mundwinkel rinnt. Im Hintergrund ist eine schemenhafte, bläuliche Fratze mit weit aufgerissenem Mund zu erkennen. Am oberen Rand befindet sich der Bandname „MORBID SAINT“ in einer großen, rot ausgefüllten Schrift mit schwarzen Umrandungen.
